Every penetration through a low-slope roof is a potential leak, and a commercial roof has many — HVAC kerbs, vent stacks, conduit, pipe supports, lightning protection. Failures concentrate here because the detailing is complex and because other trades walk on, modify and sometimes fix things to these details without telling anyone. A roof survey that does not list every penetration has not been done properly.

Yes. Unsupported conduit and pipe abrade the membrane with thermal movement and trap water beneath. Proper bearing blocks or supports are inexpensive and prevent a failure that is awkward to repair once it starts.
