TPO is heat-welded at the seams, and that is where it fails — along welds that were under-cooked at installation, at T-joints, and around penetrations where the detailing is fiddly. It is repairable almost indefinitely while the field membrane has thickness left, which is why a leaking TPO roof is very rarely a replacement conversation. The one thing that does not work is an adhesive patch: the material bonds reliably only to itself under heat.

It is a temporary measure that will lift with the first full temperature cycle. TPO is designed to be welded to itself; adhesive patches trap water underneath and often enlarge the affected area. The proper repair is to cut back to sound membrane and weld in a new piece.
Thickness is the honest measure, not age. A core sample tells you the remaining mil and whether the insulation beneath has taken water. Fifteen to twenty-five years is typical, but a roof with saturated insulation is finished regardless of how the membrane looks.
