A low-slope roof depends entirely on its drainage, and drains fail quietly. Clamping rings loosen, lead flashings crack, sumps settle away from the surrounding roof, and scuppers block with debris nobody has looked at in years. The result is ponding at best and a leak directly at the drain at worst — which is the hardest kind to trace, because water entering at a drain is already inside the build-up.

Twice a year as a minimum, and after any significant storm. It is the cheapest maintenance there is and it prevents both ponding and the overflow damage that follows a blocked outlet in heavy rain.
Codes generally require secondary drainage so a blocked primary cannot load the structure with standing water. Older buildings often lack it. It is worth checking, because the failure mode is structural rather than just a leak.
